Who is this form for?
This form is intended for students who wish to inform the University about their individual needs and request appropriate adjustments to their study conditions.
Individual needs may be related to:
- disability, including non-visible disability;
- learning difficulties (e.g. dyslexia, dysgraphia, dyscalculia);
- psychosocial disability or mental health difficulties;
- long-term or temporary health conditions that may affect participation in studies.
What information will you need to provide?
In this form, you will be able to:
- describe any barriers or difficulties you experience or may experience during your studies;
- indicate which adjustments to your study conditions may be appropriate for you;
- provide documentation supporting your individual needs (e.g. documentation confirming a disability, a medical certificate, or other relevant documentation).
Is a consultation required?
If the information provided in the form and supporting documentation is sufficient to assess your individual needs, an additional consultation is not required.
If you would like to discuss your individual situation and needs in more detail with a Disability Coordinator, you will be able to select a suitable consultation time in the form.
What happens after you submit the form?
Based on the information and supporting documentation you provide, a recommended Plan for Disability-Related Study Adjustments will be prepared.
The plan will be coordinated with your faculty, and specific adjustments to your study conditions will be agreed upon and approved. You will be informed of the final plan. Lecturers and other staff involved in your studies will be informed about the adjustments relevant to them and necessary for the implementation of the plan.
How long is the plan valid for?
The plan is generally prepared for the entire duration of your studies. If your individual needs are temporary, the plan may be approved for a shorter period.
If your circumstances change or new individual needs arise, the plan may be reviewed and updated accordingly. To request a review or amendment of your plan, please contact a Disability Coordinator at negalia@cr.vu.lt
What is important to know during your studies?
Lecturers will be informed about the adjustments approved in your plan that are relevant to their teaching and assessment activities.
Where a particular adjustment needs to be arranged individually (e.g. an assessment deadline, time, or format), please contact the relevant lecturer in advance to discuss how the adjustment will be implemented.
How is your information protected?
The information and documentation you provide in this form are confidential. Detailed information about your health condition, disability, or other individual needs, as well as any supporting documentation you provide, will remain with the Disability Coordinator.
Only the information necessary to provide appropriate study conditions and implement the approved plan will be shared with your faculty, lecturers, and other staff involved in your studies. Detailed medical information and documentation supporting your individual needs will not be shared with them.
